Little Bear Dance Center Liability Waiver

I understand that dance activities involve physical movement and carry a risk of injury. I confirm that my child and or myself am physically able to participate and take full responsibility for my own safety.

I release Little Bear Dance Center (Bold Bear Collective LLC), its instructors, and staff from any liability for injuries, illnesses, or property damage that may occur during or as a result of my participation.

In case of emergency, I authorize necessary medical treatment and accept responsibility for any related costs.

By checking the box in the form, I acknowledge that I have read and agree to the terms of this waiver. I understand the risks of participating in dance activities, confirm I am physically able to participate, and release Little Bear Dance Center (Bold Bear Collective LLC) from any liability for injury, illness, or property damage. I authorize emergency medical care if needed and accept responsibility for related costs.
