Little Bear Dance Center @ The Event Room on Main Street Liability Waiver

I understand that dance activities involve physical movement and carry a risk of injury. I confirm that my child and or myself am physically able to participate and take full responsibility for my own safety.

I release Little Bear Dance Center (Bold Bear Collective LLC) and The Event Room on Main Street., its instructors, and staff from any liability for injuries, illnesses, or property damage that may occur during or as a result of my participation.

In case of emergency, I authorize necessary medical treatment and accept responsibility for any related costs.

By checking the box in the form, I acknowledge that I have read and agree to the terms of this waiver. I understand the risks of participating in dance activities, confirm I am physically able to participate, and release Little Bear Dance Center (Bold Bear Collective LLC) and The Event Room on Main St. from any liability for injury, illness, or property damage. I authorize emergency medical care if needed and accept responsibility for related costs.
