Birthday Club Application

Thank you for your interest in our Birthday Club program.

This program is strictly reserved for families experiencing financial hardship.

Applications must be submitted at least 30 days prior to your child’s birthday.

No exceptions will be made, as this timeframe is necessary to prepare items for pickup.

Do not apply if your child’s birthday has already passed.

Late applications will be denied, and applications will not be held for the following year.

We reserve the right to request documentation at any time. Failure to provide requested documentation may result in denial of your application.
