Birthday Club Application

Thank you for your interest in the Birthday Club program.

This program is strictly reserved for families experiencing financial hardship. You must be the custodial parent or legal guardian to apply.

All applications must be fully completed and submitted at least 30 days prior to the child’s birthday. Applications that are incomplete, not filled out correctly, or missing required information will be deleted and denied without notification due to the high volume of requests we receive.

Applications for birthdays that have already passed will not be considered and will be removed.

The Birthday Club is not able to hold applications for future years, and late submissions will be denied.

We reserve the right to request documentation at any time. Failure to provide requested documentation within the timeframe given may result in denial of your application.

If approved, you will receive an approval and pick-up email from [email protected] with scheduled appointment details. Attendance at your scheduled pick-up is required. No-shows will be denied future assistance through the Birthday Club program.

For program questions, please contact us directly at [email protected]. Please do not contact Toys for Tots staff, volunteers, or the Foundation regarding Birthday Club inquiries, as they are unable to assist with program-specific questions.
